HOW TO REQUEST YOUR BIRTH CERTIFICATE

Your birth certificate is one of the most important legal documents you have to prove your identity. You’ll often need it to apply for other important documents. Here’s how to request birth documents for you, your child or someone in your care.

WHEN YOU’LL NEED YOUR BIRTH DOCUMENTS


You’ll need to provide your birth documents when you’re applying or registering for:

  • School

  • Your Social Insurance Number

  • A driver’s licence

  • A passport


REQUESTING YOUR BIRTH DOCUMENTS


Anyone born in Alberta can request birth documents for themselves or their children. The process below outlines the steps you’ll need to take if you’re applying from within the province of Alberta. To learn more about applying for birth documents while outside the province or about the different applicants allowed to order birth documents, consult the government of Alberta’s website.



1. DECIDE ON WHICH TYPE OF DOCUMENT YOU’LL NEED


Birth certificates issued in Alberta come in only one size, but you do have options as to the information included in the document. You can choose to order a certificate with only your personal information, one with personal information about you and your parents, or certified copy of your complete registration of birth.



2. COMPLETE THE APPLICATION FORM


Confirm your eligibility to apply for birth documents, and then download and complete the Application for Birth Documents form. You can also find this form on the government’s website, or visit Registrations Are Us to pick one up in person.



3. RETURN THE APPLICATION FORM TO AN AUTHORIZED REGISTRY


Your completed Application for Birth Documents form must be brought in-person to a Registry Agent authorized and recognized by the government of Alberta. When you drop off your application, you will be asked to provide a piece of valid government-issued photo ID such as a driver’s licence and also to pay the application fee.



AFTER YOU APPLY


After you submit your application to the registry agent, your birth documents will be mailed to the address indicated on your application within two to three business days or will be available for pick up in the office.
